Scheduling & Cancellations
CIMS has a team approach to patient care. Our team of experts look forward to working closely with you toward your renewed health. Scheduled appointments are reserved specifically for you and we value your time. We diligently work to start and end your appointments on time.
Cancellations/no-shows can cause significant interruptions to our clinic. A cancellation charge will occur when notice is received less than 24 business hours (Monday-Friday) or if there is a no-show to an appointment.
Cancellation fees are at 100% of fees for service.
We do require a credit card to schedule appointments and will charge the card provided in event of late cancellation. New client registration forms must be completed at least 72 hours prior to your visit.
Current Fees & Pricing
All visit fees are due at the time of service. Chiropractic visits are applied in 15-minute increments with exception of initial or follow up evaluations.

Dr. Osar is licensed in Wisconsin and regularly sees patients worldwide through virtual visits.

- Follow-up visits $115-$215)
Manual Therapy with Jenice Mattek
- New client evaluation: $175
- Sessions Range from $80-$130
Massage Therapy with Jill Leary
- New client evaluation: $175
- Sessions range from $80-$160
Personal Fitness with Sara Fisher:
- New client evaluation: $175
- Sessions range from $80-$160
Insurance | Payment Policies
We are not involved with insurance companies, nor do we create or submit any claims to insurance companies.
We accept American Express, Visa, Mastercard and cash.
A credit card is required to hold your appointments.
If you are unable to pay in full at the time of the appointment, please let us know and we will do our best to work with you.
